Screening Activity Report (SAR)
SAR provides screening data for breast, cervical and colorectal cancers to help physicians improve their cancer screening rates and appropriate follow-up.

If you are already registered with OneID and would like to access your SAR please click here.
OneID Registration
To access your SAR report, a OneID username and password is required. By registering for OneID, primary care providers and nurses have access to Ontario Health services including the Screening Activity Report (SAR).
Physician Enrolment Model (PEM) providers can self-register for OneID through the College of Physicians and Surgeons of Ontario (CPSO) website.

Physician-Linked Correspondence (PLC)
Physician-Linked Correspondence (PLC) includes physicians' names in their rostered patients' cancer screening letters from the organized screening program. Physician-Linked Correspondence has been shown to motivate individuals to get screened for cancer.

Physicians are required to provide their signature to show that they understand and acknowledge what they are opting into and their responsibilities. Physicians are asked to provide consent for all 3 cancer screening programs at once. By doing so, they will automatically be enrolled when physician-linked correspondence expands to the Ontario Breast Screening Program (OBSP) and the Ontario Cervical Screening Program (OCSP). We will give you details about this expansion before it takes place.
